Ok so how should I put this on a job application?

Ok so how should I put this on a job application?

what should I put as reason for leaving job for the following?
I worked at fastfood from 2006-2007 and gave a two weeks notice to quit (I will put better oppurtunity)
than from 2007-april 2009 i had another job i quit it a job with out giving a notice :) (i know bad me) what should i put as reason, i just called and said i wasnt coming back in and they were like ok, are you sure that means that you can't ever come back to work for us and i said yes. Should i put the reason as just i wanted to try something new? or what? by the way I know I shouldnt have done that.
I also had a new job offer already and I worked there from April 2009-Aug 2009 than i went on maternity leave and was suposed to come back at some point (not exactly sure when lol) but i moved so i found a job closer to where i lived and started that in october and never talked to the job from april to aug again. and still work at the october one today. but i am applying for another job and hopefully this will be my last one until i finish school. lol. but what should i put does this sound fine or is there a better way to put it?
2006-2007 better oppurtunity??
2007- april 2009 wanted to try something new?? (by the way what i do now is very similar to this job just a different company)
april 2009- august 2009 moved??
And oct 2009- current- currently employed??

does that sound good? i know i should have quit the middle 2 properly but i didnt. if they ask in an interview i will just be honest...lol... but do those reasons sound good?
